POLICIES

Minors Forms

Parents/guardians and participants will receive permission forms via DocuSign shortly after registration is complete. All forms must be returned by the DocuSign deadline in order to participate.


Photo/Video Release Forms

All participants (minors and adults) must complete the Photography/Videography Release Form which will be delivered via DocuSign. The Community Music School reserves the right to take photographs and video of faculty, staff, and participants engaged in teaching, rehearsals, performances and other activities. These assets may be used in university publications such as websites, digital signage, flyers, media advertising, admissions recruitment, development brochures, etc.


Attendance

Regular punctual attendance for lessons, classes and ensembles is essential for every participant's success. Prompt arrival will facilitate an on-time start and full, uninterrupted instructional time. Registering participants are reserving time and committing to a session of lessons/classes/ensembles, and as such, they are expected to attend all scheduled lessons and classes. If a participant needs to miss a lesson, class or ensemble, they must notify the instructor 24 hours in advance. Last-minute cancellations PRIOR to the lesson are accepted only for participant illness and family emergencies.

No refunds or tuition adjustments will be given for missed lessons, classes, and ensembles. Instructors are not obligated to schedule make-up lessons for participant absence. In case of instructor cancellation, a make-up lesson will be scheduled. Participants will not receive make-ups, refunds, or tuition adjustments for missed classes and ensembles. If a make-up lesson fails to be arranged after the teacher’s cancellation, please contact the teacher in writing to remind them and reschedule. If communication fails and no make-up lesson is given, notify Angela Heck Mueller at heckmueller.1@osu.edu.


Withdrawal/Refund Policy 

Participants are reserving time and making a commitment for an entire session of lessons/classes/ensembles. Refunds will not be given if a participant decides to withdrawal from the program.


Emergency/Weather Policy

In the event of a Level 2 Snow Emergency in Franklin County or in case of an emergency closure, lessons, classes and ensembles will be held online.


Staff Background Checks

All CMS instructors must complete a background check with The Office of Human Resources. The university must receive background check results, and an individual must be cleared to work with youth before an activity or program starts.
