Required forms for students participating in Outreach events
School of Music outreach programs are required to collect two (2) or three (3) permission forms. Download, print out, complete, sign and send the required forms to the attention of the designated contact person according to the instructions on the event's web page.
REQUIRED for ALL programs:
Youth Summer Music Programs
In addition to the Parental Permission and Photo Release forms above, the Standards of Behavior form (below) is required for each of the Youth Summer Music Programs. All YSMP programs require all three completed and signed forms to be mailed to the School of Music at the address noted below OR they may be scanned and emailed to the YSMP registrar at YouthSummerMusic@osu.edu.
