Credits, Curricula and Candidacy

Graduate Individual Study Request Procedures
- Confer with the faculty member directing the individual study and determine the:
- Semester (include the term)
- Year
- Course Number (6193 for master’s and 8193 for doctoral)
- Description of the project — In 100 words describe the learning objectives, means of achieving them, and outcomes of the course.
- Discuss with Advisor if this course is to be used as a substitute for another. Notate the substitution on the form.
- Student initiates the Graduate Individual Study Form in DocuSign.
- Student will input the email addresses for both the Instructor and the Advisor. In a situation where the Instructor is the Advisor, input that professor’s email address in both requested categories.
- Student will complete their portion of the document. Once finished, the document will automatically be sent to the Instructor and Advisor for signatures.
- Once the form is complete, the student will be notified and will receive a copy of the completed form for their records. Upon completion, the form will be automatically sent to Kurt Phelps, graduate studies coordinator.
- Students will be registered for the course listed on the form, and will receive an email confirming their enrollment.
DMA Recital and MM Piano Concerto Recital Approval Form Instructions
Graduate students performing either of these recitals must complete this form, in addition to all other requirements found in the Student Recital process.
- Before starting the DocuSign process, you must have a committee formed. For details about who can serve on the committee, please see the School of Music Graduate Handbook.
- Check all committee members' names and email addresses. Be certain to use only osu.edu email addresses.
- You initiate the Graduate Recital Approval Form in DocuSign.
- Be certain to list your advisor under Primary Advisor, as that individual will mark Satisfactory/Unsatisfactory after consultation with your committee.
- All other committee members may be listed in any order after the advisor.
- Once all committee members have signed, the Graduate Studies office will receive a copy which will then be placed in your electronic file. It is recommended that you also keep a copy for your records.
Procedures to Waive MUSIC 6786: Music Research Methods and Bibliography
Students must gather all required materials before starting the Bibliography Course Waiver Request Form.
Gather Required Materials
- Justification Letter explaining how your previous graduate course covers the content of MUSIC 6786
- Syllabus from your Master’s program research course
- 1–3 Sample Writing Assignments from that course
- Master’s Transcript showing completion of the course
Complete the DocuSign Form
- Start Bibliography Course Waiver Request Form
- Input your name and your advisor’s name
- Upload all required documents
- Submit the form when finished
Review and Notification
- Your materials will be reviewed
- You will receive a copy of the completed form with the decision for your records
