This is an open call for participation. 

Please compile the following information to complete the registration form. Submit the form no later than Monday, October 6.

  1. Title/composer (exactly as you wish it to appear in the program)
  2. Exact timing (please limit to 6 minutes or less)
  3. Ensemble name (exactly as you wish it to appear in the program)
  4. Instrumentation (for staging purposes)
  5. Performer names (exactly as you wish them to appear in the program)
  6. Any special staging or sound needs (exact # of chairs, stands, microphones, etc.)
  7. Short description (for programming purposes)
